- I/we, the undersigned, understand and acknowledge that participation in a recreational activity can be hazardous and I/we realize that no one should enter into recreational activity unless the participant is medically able.
- I/we assume all risk associated with this including but not limited to: falls, contact with other participants or equipment, effects of weather, equipment failure, and condition of playing area.
- I/we fully understand it is my responsibility to ascertain if this specific activity contains other elements of risk that could prove harmful to a participant. Having read this waiver and in consideration of acceptance of entry into the program, I and anyone entitled to act on my behalf waive and release Ocee Park Athletic Association, its sponsors and successors, the City of Johns Creek Department of Recreation and Parks and their representatives from all claims and liabilities of any kind arising out of my child's participation in this activity.
- I certify that the birthdate and age of my child as stated on the registration form is correct. I understand that I may be required to submit proof of both residency and birthdate to Ocee Park.
- Any and all personal information received via various communication means to include, but not limited to email messages and telephone are to be used in accordance with OPAA's privacy policy governing use of such information. Personal information attained by any means will be used solely for the purposes of OPAA organizational business related to communications of OPAA events and will be purged after each season from personal address books or any other retention methods. Use of personal information for any purposes including solicitation or for personal gain during or after the conclusion of each season is prohibited. Failure to adhere to this policy will constitute a violation of OPAA privacy policy and the Member Code of Conduct, and may lead to disciplinary actions including suspension and up to expulsion from the program.
- Withdrawal requests may be made via email to registrar@oceepark.com. Any withdrawals requested before the withdrawal deadline will be entitled to a refund of fees paid minus a $25 non-refundable registration fee. Any withdrawals requested after the withdrawal deadline, will not be entitled to any portion of a refund.
- OPAA is an independent volunteer organization which uses Ocee Park field space in partnership with the City of Johns Creek Parks and Recreation. As such, the City of Johns Creek has imposed a 50% non-residency fee for those players that do not reside within the city limits of Johns Creek, GA or Alpharetta, GA. You will be asked if you are a resident on the next screen and the non-resident fee will be calculated as appropriate. Residency records will be verified by the city of Johns Creek and any misleading residency information or omission of residency fees could result in suspension or expulsion from the program.
